Patient Forms

Upon entering our office, one of our staff members will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.

Consultation

Next, you will have a consultation with Dr. Audra Hewett to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Audra Hewett to learn more about you, your condition,

and expectations to determine how chiropractic care can meet your goals.

Comprehensive Examination

After your consultation, Dr. Audra Hewett will perform a complete musculoskeletal and nerve examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well.

However, nothing will be done in our office without your consent.

X-Ray Studies

If you have any MRI images or x-rays, please bring them in on the first visit, otherwise we do not do x-rays in house.

Report of Findings

Once the information is collected and examinations are performed, Dr. Audra Hewett will answer any questions including:

  1. How can you help me?

  2. How often do I need to come in?

After reviewing your health history, goals, and examining your spine and X-rays, Dr. Audra Hewett will discuss recommendations and notify you if your condition requires care with other providers.

Dr. Audra Hewett will provide the best treatment and wellness program for your needs.

Treatment

At the conclusion of this initial appointment, you can choose to experience your first treatment. This may include spinal adjustments, physical therapy, physiotherapy, supplements/nutritional advice and/or soft tissue massage. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.

Wellness Program & "NextGen" Packages

Prior to leaving, the doctor will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches.

If you desire, our wellness team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.

We will offer many package plans for you and your family to receive care from Next Generation Chiropractic & Wellness. Please Call for our pricing. These package plans may be discussed with our Office Manager.

WHAT CONDITIONS DO YOU TREAT?

Why will a Chiropractic Doctor do a Comprehensive Exam?

Chiropractors are trained in examining the joints, bones, muscles and tendons of the spine, head, extremities and

other areas of the body with the purpose of noting any misalignment, tenderness, asymmetry, defects or other problems.
Chiropractors are trained to perform a variety of neurologic tests (nerve root compression/tension, motor strength, coordination, deep tendon and pathological reflexes, etc.) and are skilled in performing orthopedic, cardiovascular and many other common examinations. Chiropractors are trained to assess range of motion, stability, muscle strength, muscle tone and other assessments with the lower back. Chiropractors are trained in use of diagnostic studies and tools such as radiography (X-rays), laboratory diagnostics and neuro-diagnostics. In the assessment of lower back pain, differential diagnosis utilizing a "triage" concept of classifying low back injuries into one of three categories helps to guide the doctor of chiropractic.

These categories of chiropractic diagnosis include:

1. Potentially serious: tumor, infection, fracture, major neurological problem (cauda equina), local open wound or burn,

prolonged bleeding (hemophilia), artificial joint implant problems, pacemaker problems, joint infection

2. Nerve problem: when the nerve root in the low back is pinched or compressed, causing a radiculopathy (sciatica).

Typical causes of nerve root pinching include a lumbar herniated disc, spondylolisthesis, spinal stenosis

3. Non-specificmechanical back pain in the lumbar spine. This type of lower back pain is the most common

presentation, and includes pain for which there is no identifiable cause.

With chiropractic diagnosis of potentially serious injuries, the chiropractor will typically refer the patient to a relevant medical specialist and possibly a surgeon, and as appropriate the chiropractor may co-manage the patient's care with other back pain specialists. With this classification, chiropractic manipulation is typically avoided over the relevant anatomy.
